Philippine Nursing Board Exam Checklist
Nursing Board Exam/Nurse Licensure Examination (NLE) Checklist
Before the day of the nursing board exam, make sure you already have:
1. Read and understood the Examinee’s Self-Instruction Sheet
2. Prepared your clinical or school uniform. Refer to: PRC-BON Resolution No. 20 Series of 2009: Amendment to Board Resolution No. 17 Series of 2009 dated July 31, 2009 on Prescribing a Dress Code for Examinees in the Nurses Licensure Examination
3. Bought or acquired a calculator that is on the list of non-programmable calculators allowed to be used in licensure examinations. (PRC Memorandum Circular No. 2008-19)
4. Visited or know exactly where you are assigned (name of school and room number) to sit for the board exam (room assignments are usually released 3 days before the 1st day of the board exam). You have to report to the test center before 6:30 a.m. on the first day of examination to verify your room and seat number. Remember that late examinees will not be admitted.
Requirements on the day of examination:
1. Notice of Admission (NOA) Form (to be brought EVERY examination day)
2. NLE Application stub
3. PRC Examination Fee Official Receipt
4. Two or more no. 2 pencils
5. Ballpoint pens (black ink only)
6. One (1) piece metered-stamp window envelope
7. One (1) piece long brown envelope
8. One (1) piece long transparent (non-colored) plastic envelope (to keep above items)

whats the NLE application stub?
@kenichi – it’s that small piece of paper that has your application number for the board exam. I believe it was torn off from that card-type application form which you submitted to the PRC.

how can we get a NLE stub if we apply last Sept 2010…?
because i have a NOA and PRC receipt only.

ndi po ba same lng yun sa NOA kc nandun ang application number e.. jsut asking… tinatanung ng kaibigan ko e.
@Carlo – NOA is that blue-colored paper where the room watchers would sign during the examination days. You would put your Application Number in it so you can refer to it when filling-out an identification form on the day of the exam.
gud pm ma`am rona. nag apply kmi ng son ko nag file kmi sa prc last sept. and after we finished the filling wala clang bngay samin na NLE stub at ok na daw un at na complete na nmin lahat requirements. pano un mam rona, you mean kpag wala un hindi makaka take ng exam ang anak ko? kailangan pba nmin bumalik sa prc to get that stub? tnx po.
@lhet – if you would click the link above leading to what that application stub looks like, you would see na isang pirasong papel lang yun na nakalagay ang application number para sa board exam. if i am not mistaken, either sa board exam na niya mismo makukuha yun or hindi na kailangan yun talaga. hindi rin kasi updated ang website ng PRC na kung saan galing ang listahan na nilagay ko. no need to worry about this as long as nasa listahan ang naman ng anak niyo po.

bakit wala kami ng Application stub na yan… this is my 2nd take pero wala namn kinuha last time..

@Civor – it’s just a small piece of paper that has your application number for the board exam. if you didn’t have it during your first take then you were able to take the board exam, then it’s fine that you don’t have it now. PRC’s list is not updated which is where that list came from.

@nath – it’s best that you copy your Application Number to your NOA form because you will use it when you fill-out an identification form during the examination.
is the chn uniform acceptable?
@marie – I am not sure if the CHN uniform is allowed. According to the said Resolution:

@mafe – I guess pwede na yun as long as puti siya and it’s considered a “clinical uniform” since sabi mo nga ginagamit mo siya as a volunteer.
Ms.rona ung envelope po ba wla ng nakadikit na stamp or anything?

@gian – baka meron doon sa testing center mabibilhan. maraming magbebenta doon ng pencils at envelopes at ng kung anu-ano pa na para sa board exam.
pano ko malalaman ano license number ko eh di ko pa nakukuha license ko.is there any other means?
@feye – check your claim stub. The registration number written on it will be your license number too.
hello po.. tanong ko lng kung pwede kaya white blouse and black pants? kc mtgal na since last ako magtake ng board at hndi na kasya mga uniform ko ngaun..

@ella – kung napayagan ka na mag take ng exam dati na naka scrub suit, then I guess pwede ulit na naka scrub suit ka ulit ngayon kasi pwede naman i-consider na “clinical uniform” yun.
mam rona bat po ganun??? wala nmn bngay smin na stub???? nung sept pako nakapag file nung after ko mag file nag ask pako kung ok naba kung kumpleto na sbi ok na d2.. ngaun ngulat me my stub n gnyan 1st taker po ako baka po pag dating sa examination day eh hanapan kmi nyan, wala nmn po smin bnbgay n gnyan huhuhhuh!

@aiza – you have to personally apply at the PRC to sit for the NLE.
mam baka pwede pong ipadala nila sa akin d2 mga form taz fill up q po lahat then isend q po sa pinas..is that possible???
@aiza – I just called PRC to confirm this. The test-taker is the ONLY one who can apply to be able to sit for the NLE. No representatives will be entertained.

ma’am rona, gud eve ask ko poh about sa cases, mga cases ko poh kc year 2003’04’05 pa poh at grad poh ako 2007 kaso hindi poh ako agad nag brd exam, ngaun plang poh ako magffile pwede pa poh ba mga cases ko o kelangan ko poh mag duty ulit para sa bago na cases ko poh, thank you.
@kim – what I know is you do not have to undergo duty again. Nag-duty ka na and you already have completed the requirements by the PRC-BON. It doesn’t really matter kung expired na ang mga PRC number nila dahil cases mo sila nung 2007 (when their PRC licenses were not yet expired). When in doubt, call PRC. Best of luck to you.

i’m a second-taker for this coming december 2011 nle. nag take ako nung dec 2010 nle. di ko natandaang binalik yung NAF. I only have the NOA. Ang question ko po is that, makakapag file ba ko kahit walang NAF? Or ok lang kahit wala anyway bibigyan naman ako ulit?
You will have to fill-out another NAF when applying to take the board exam again.
hi mam! itatanong q lng po,. Kpag nkpag file kana sa prc and bngyan kna ng NOA with official receipt makakapag take kna ng exam for december? Or pwede ka nlang idisqualified on the exam day due to some reasons ,. Possible ba mangyari un?
