hi pwede ask about application form. sa name, ano po ang sa middle name, tapos may middle initial pa? pareho lang ba yun? dalawa po kase name ko so i am confused. can you give me a sample? thanks
@che – kung may second name ka, yun ang middle name. Middle initial (M.I.) will be your mother’s maiden last name.

Sa middle name, yun yung kung may second name ka pa like Joseph Anthony. Anthony ang middle name mo. Then yung middle initial, yung 1st letter ng Middle name (surname) mo — as in your Mother’s LAST NAME before she was married.

ate rona, eto na po ba ung dapat iprint? kasi po may nakasulat na webcopy sa likod at REPRODUCTION IS NOT ALLOWED. e ung sa mga classmates ko po nung july takers pede naman po iphotocopy.. help po. kung hindi po ito un, san po pede idownload NAF. maraming salamat in advance po.
magtatanung lang po ako,di ba po pwede i download yung Nurse’s Application Form (NAF)sa wedsite ng PRC (www.prc.gov.ph),pwede po ba sa 8.5 x 13 o dapat talaga na sa 8.5 x 14 ang laki nung pagpipritan na bond paper? Tapos po,pwede na din po ba yun ang ipasa sa PRC pag a-apply o dun mismo sa PRC nakuha nung NAF?
hello po, bakit hindi naka lagay dito sa schedule kung kilan ang starting ng filing nila?? bakit ang deadline lang?? di gaya ng napolcom at civil service kompleto ang detalye! kaya tuloy ako di nakapag file. taga iligan city ako, tapos nagpunta ako prc sa CDO nagtatanong lng sana kung pwede pa makahabol mag file as retaker pro PUYA GABA FOR WINDOW-1, BABAE PA NAMAN SANA, PERO NAPAKASAMANG UGALI!di sya bagay dun.
