Nurse Licensure Examination (NLE) Requirements for First-Timers and Repeaters.


FIRST-TIMERS:
1. Duly-accomplished PRC Nurses’ Application Form (NAF)
2. NSO-issued Birth Certificate (original and photocopy)
3. NSO-issued Marriage Contract (for female married applicant; original and photocopy)
4. Transcript of Records (TOR) that indicates and has the following:

  • degree/course
  • exact date of graduation
  • Special Order (S.O.)/exemption/accreditation/deregulation status
  • scanned picture and;
  • the remark: “FOR BOARD EXAMINATION PURPOSES ONLY”

Note:
-Graduates of government schools and institutions/programs accredited by recognized accrediting agencies under the FAAP are exempted from S.O.
-Graduates of New Schools/Degree/Programs must submit School Recognition and/or Permit to Operate.

5. NOTARIZED Operating Room and Delivery Room Cases

  • Operating Room: Major Scrubs (5); Minor Scrubs (5)
  • Delivery Room: Handled (5); Assisted (5); Cord Dressing (5)

6. NOTARIZED Summary of RLE (Related Learning Experience)

7. Examination Fee:

  • Baccalaureate Degree: Php 900
  • Non-Baccalaureate Degree: Php 600
  • Removal: Php 450

8. 4 (four) pcs. passport-sized pictures in white background with complete name tag
9. 1 (one) piece window mailing envelope with metered postage stamp
10. Current Community Tax Certificate (Cedula)
11. Other specific requirements (if there’s any) as required by the Commission and/or the Professional Regulatory Board.

 

SECOND-TAKERS (REPEATERS):
1. Duly-accomplished PRC Nurses’ Application Form (NAF)
2. NSO-issued Birth Certificate (original and photocopy)
3. NSO-issued Marriage Contract (for female married applicant; original and photocopy)
4. Transcript of Records that indicates and has the following:

  • degree/course
  • exact date of graduation
