Nurse Licensure Examination (NLE) Application (Step-by-Step Instructions)
The Professional Regulations Commission (PRC)
Step-by-Step instructions on how to apply for the Nurse Licensure Examination (NLE).
STEP 1. Secure a Nurses’ Application Form (NAF) from the PRC Customer Service Center (CSC) and accomplish the same.

STEP 2. Submit the Nurses’ Application Form (NAF) for processing and evaluation at any of the processing windows together with the rest of the other Nurse Licensure Examination (NLE) Requirements.
Secure the following from the processor upon approval of your application:
STEP 3. Pay examination fee at any of the cashier Windows and claim the Official Receipt (OR).
- Baccalaureate Degree: Php 900
- Non-Baccalaureate Degree: Php 600
- Removal: Php 450
STEP 4. Pay for metered documentary stamps and mailing envelope with metered documentary stamps at the CSC.
(Admin Note: This is for the Nursing Board Exam Report which will be sent to the address give by the examinee after the nursing board exam result has been released. )
STEP 5. Fill-out the PERRC and NOA form. Indicate the Application Number, OR number and the date on it, and amount paid on the NOA form.
STEP 6. Proceed to any of the Issuance Windows (Window 9, 10, 11, 12) for final review of qualifications and documents and issuance of NOA form, Program of Examination, Examinee’s Kit and Self-Instruction Sheet.

STEP 7. Keep the NOA form and the OR. Bring them on the first day of the examination. For further information, read the General Instructions at the back page of the NOA form, Program of Examination, Examinee’s Kit and Examinee’s Self-Instruction Sheet.
STEP 8. Bring the NOA form when verifying with the PRC the school/building where you are going to sit for the nursing board exam. School/building assignments will be posted at the PRC premises two (2) or three (3) working days before the examination date.
Reminders:
- No applications shall be accepted after the deadline set by the PRC.
- Do not apply if you are not qualified. Any payment made will be forfeited.
- Applications whose entries with respect to NAME, DATE OF BIRTH and GENDER differ from entries in the Birth Certificate, Transcript of Records (TOR), and other required documents WILL NOT BE ACCEPTED.
- Affidavit of discrepancies will not be accepted.

What if i forgot to have an issuance, can i go to Baguio on September 29? because i have a class on September 28,but i already paid the application..
@Joanne Buenaventura – Go to Baguio on September 29 to what? You have to have with you the NOA, Applicant’s Stub, Program of Examination, Examinee’s Guide and Self-Instruction Sheet.

@Nicole – you will have to apply again but you won’t be submitting the OR and DR cases anymore.
all same requirements pa din po ba (except the OR and DR cases) po right? And when po kaya start ng filing for the July 2011 boards? Will my deadline be under the new takers or the repeaters? thanks po:))
@Nicole – that’s correct. same requirements and process of application except that you won’t be submitting anymore your OR and DR cases. I believe you will be under new takers as you never really got to take the exam. You will have to verify this too during your application though just to be sure.
The PRC hasn’t released any official schedule yet for 2011. I’ll post it here though as soon as it comes out.

@Nicole – which number are you using to call them? It’s not a surprise that it’s hard to call them especially now that the board exam is nearing. You may call them using the numbers found here: PRC Central and Regional Offices Telephone Numbers and Addresses

ask ko lng po. pwede pa rin po ba akong mag apply for NLE kahit 2 years na ang nakaraan? nabasa ko po kasi sa result na binigay sakin nun na “Removal Examination shall be taken within two years after the last failed examination”.
ask ko na din po kung sakaling pwede magreexam pwede po ba akong magapply sa Launion or Baguio. sa Manila po kasi ung first application ko nun maam.

@dice – are you recommended to take the removal exam ba? If you are, pwede ka kumuha ng NLE (removal) as long as pasok sa within two years na binigay nila for you to take the removal exam. I am not sure kung doon ka dapat where you originally took the board exam na mga a-apply para sa removal though. @dice – it’s best that you inquire directly with PRC regarding your situation para di masayang ang pag-apply mo. Di ko din kasi alam kung pwede mo i-repeat yung entire exam kung removal lang ang recommended for you. You can ask them too if you can take the NLE after 5 years since you took it the first time.

Hello. I’m hoping someone here can verify this for me. A friend of mine says that to become licensed she needs:
… have the review 17,000pesos and the application for board 2,000pesos…

@Alice – to be a licensed nurse (RN), you will only have to PASS the nursing board exam (NLE). Enrolling in a review center is ENTIRELY just an OPTION. It’s NOT A REQUIREMENT. Also, there’s no guarantee that you will pass the nursing board exam if you enroll in a review center. Take note that the application fee to apply to sit for the board exam is ONLY Php 900 and NOT Php 2,000.
I am married in usa, Do the prc stl need nso verified marriage cert? Pnu un? Chaka 2009 pko graduate, valid pa b ung 555 ko?
@Aina – if you are currently using your married name, then the PRC would need a marriage certificate to verify that you are indeed married. I have to ask, what is the 555?

@rauf – you can call PRC Davao first to ask if they are already accepting applications for the July 2010 NLE. Call them first para di masayang ang pagpunta mo doon.
mam rona, gusto ko i.ask kung paano po kung mali ung entry ng 2 letters sa middle name. Ma, question pa rin ba un khit ang tama ung ginagamit na middle name sa mga school records at TOR? nagwowrry kc ako na bka hindi makapagtake ng board exam ung kapatid ko this july. Pde po ba ako pmunta dun sa prc para humingi ng approval letter nila?
@jen – yes baka magkaproblema siya sa application niya. kailangan, kung ano yung nasa documents na pinasa (TOR, Birth Certificate), ganun din ang nakalagay sa application form.

Hi Ms. Rona, 2yrs na kong grad and this coming July ko balak itake yung exam. Do you happen to know or have any idea po ba if how much yung cost magpanotaryo nung cases & RLE summary if ganun na katagal? Sa inquiry kasi namin nasa 10k daw.. aside from that, may special na requirement kaya na hihingin ang PRC in this case o wala naman? Do I need to take any refresher course din po ba? @Tere – who told you that notarization of your cases and RLE will cost you 10,000 pesos? It doesn’t cost more than 50 pesos per page to have it notarized. Somebody’s scheming you. No special requirement even if it has been 2 years since you graduated.

@julius – you can’t take the NLE without the NOA form. You have to go back to the PRC and ask for it.

my only problem is that on my prc forms, I have a middle initial that shouldn’t be there. I was able to fix it in the school by providing my NSOBC and making affidavits…I hope it’ll be accepted by PRC cause its just a middle name or middle initial, besides there is no law regulating the use of middle name..thank you, please respond

Can I ask po If pwde pa yung 555 cases ko for december 2011 if expired na po yung PRC license nung dean and level coordinator namin this june?? completely sign na po siya and may notary na.. I decided po kasi na sa december na lang ako magttake ng exam… Is it valid pa din? Or I need to redo it again? Tnx po and godbless…
@Mae – what I know is you have to have it signed again when the signatories renew their licenses. You may want to verify this personally with PRC too just to be sure.
hey i have a friend he’s a repeater and he was not able to file to the repeaters deadline can he still apply to PRC??…or can he apply to the said deadline of the firstimers?…thankyou
@kit – As of this writing, there’s no advisory from the PRC website that the deadlines they have set will be extended. To be sure, call PRC to inquire about this.

ma’am good day po! cases ko po nakuha ko year 2009 pa. i heard na valid lang ito for 2 years? i plan to take the board next year. does that mean i have to recomply the cases again?
@m.a. – who says that it would be only valid for 2 years? Those are your cases and there’s no need for you to complete them again.

About sa cases ko..kc mostly i computerized the year when i was graduated.but then sa nursery cases ko kc nakalimutan ko ilagay ung year wen i graduated. Plano ko sana sulatan ko nlng xa..is it still acceptablr by the PRC staffs?:) thnx..

@angel – you are not a repeater since you never took it in the first place. pero since wala naman ibang category sa NAF na pwede mo sabihin na re-taker ka kasi di ka nakapag-exam, yun na lang din ang i-check mo. refer to the requirements above.

Look for ay other documents such as your receipt.
Ma’am.. Please po reply po kayo… Nakapagfile na po ako sa PRC, tinangap na po nila, may NOA na rin po ako kaya lang po nung nakita ko yung application form na sample, hindi ko po naindicate yung ‘Status Code’ pero tinangap na po nila and then nag ‘thumbs up’ po sakin yung nasa window…

PRC personnel na mismo ang nag approve ng application mo. I don’t think you have to worry about this anymore.
