To request for this certificate, go to Window A/B. Look for the PRC Action Sheet Form. Fill-out the action sheet form and pay the request fee at the cashier:
Express request = 1 hour (PhP 200)
Regular request = 1 month (PhP 75)
Bring the action sheet back to window A/B and return on the date indicated on the claim stub.

hi. two of the requirements for rn heals are board rating and board certificate. the one shown above is a certificate of board rating. so iba pa po pala ung board certificate? pls help.
pwede po ba akong mag.request ng certificate of board rating sa manila kahit dito ako sa prc baguio nag.register and ng.take ng board exam dito cabanatuan ? kailangan ko na po kasi requirement po.
Hi, nagpaaunthenticate ako ng board rating tapos kelangan iiwan ung original copy, tapos pinadeliver ko thru dhl , I received it but prob is wala po ung original copy, I tried calling their hotline pero busy lagi eh pls help me
gud pm po..anu tawag nung blue na papel na pinadala ng prc??yung may breakdown ng rating per exam??kelangan ba yun para sa red ribbon processing sa prc?thnks!
kahit saang PRC branch ba pwede magrequest ng certificate of board rating or dapat dun kumuha sa PRC branch kung saan nag-apply for the board exam? thank you po
