Nurse Licensure Examination Review Questions
The Philippine Nurse Licensure Examination (PNLE) is a 500-item, multiple choice, 2-day exam given by the Professional Regulation Commission (PRC) – Board of Nursing (BON) to test basic nursing level competency and subsequently for one to become a Registered Nurse (RN).
It is divided into five (5) PARTS that contains one hundred (100) questions each. They are:
1. Nursing Practice I: Basic Foundation of Nursing and Professional Nursing Practice (100 items)
2. Nursing Practice II: Community Health Nursing and Care of the Mother and Child (100 items)
3. Nursing Practice III: Care of Clients with Physiologic and Psychosocial Alterations, Part A (100 items)
4. Nursing Practice IV: Care of Clients with Physiologic and Psychosocial Alterations, Part B (100 items)
5. Nursing Practice V: Care of Clients with Physiologic and Psychosocial Alterations, Part C (100 items)
The questions of these five (5) PARTS of the NLE are based on the knowledge, skills and attitude of an entry-level nurse in the following five (5) MAJOR SUBJECT AREAS:
1. Fundamentals of Nursing including Professional Adjustment
2. Maternal and Child Health Nursing
3. Community Health and Communicable Disease Nursing
4. Nursing of Adolescents, Adults and Aged
5. Mental Health and Psychiatric Nursing
The following five (5) AREAS OF KNOWLEDGE will be integrated in each of the five (5) MAJOR SUBJECT AREAS mentioned above:
1. Anatomy and Physiology
2. Pathophysiology
3. Pharmacology and Therapeutics
4. Nutrition and Diet Therapy
5. Parasitology and Microbiology
Furthermore, the PNLE will test competencies in relation to the eleven (11) KEY AREAS OF RESPONSIBILITIES contained in the National Core Competency Standards for Filipino Nurses by the Board of Nursing (BON). They are:
1. Safe and quality nursing care
2. Communication
3. Collaboration and teamwork
4. Health education
5. Legal responsibilities
6. Ethico-moral-spiritual responsibilities
7. Personal and professional development
8. Management of resources and environment
9. Records management
10. Research
11. Quality improvement

to Ann, Lain and alvin. I just took the NLE last december 2011.
My advice is take time to review everything that goes around in NP1. Most of us consider it as the killer part of December 2011 exam. It is all about research and basic knowledge with regards to fundamentals (mga procedure like catheterization, ngt, ogt etc…). Madali sya kung iisipin pero madalas kasi hindi nasusunod yung standard procedure sa clinical duty pero when it terms of taking the exam we need to consider such things to be performed in its standard procedural way in NP1. Mahirap sya infairness yan ang lowest ko! lol…
saka may tanong na what ung may pinaka mataas na potassium content kung (orange ba, apple, papaya or banana) number 1 yan sa NP1…
in NP 2, may computation na lumabas ung about sa mga mortality, morbidity, specific gravity rate sa CHD yan…. sa IMCI madami din ang nakaka pressure is sobra haba ng choices kaya hindi tatalab na piliin ung longest choice na given!!!! hahaha technique kasi natin yan dba!!!
NP 3, this is my highest napaka rami lumabas about (parkinson dse, hypo and hyper thyroidism and para thyroidism, DM, types and differentiations ng ulcer, STI, transvertical therapy, ONCE NA MAKABISADO NYO ANG PASIKOT SIKOT NG MGA YA GREAT CHANCES IN PASSING THE EXAM….
NP4, same as above mentioned medyo mas mahirap lang ng konti
NP 5, madali lang to! susukatin lang ganu ka ka bait na nurse… pero more on ADHD, IQ ranges ng tao, bipolar nako!, anxiety etc….

hi first thanks sa mga payo nyo lalo na po kay Niko Lae Abdon, RN. hmmp, sabi ko magseself review ako pero d ako makaconcentrate dito kasi me now as AUS. nagtour pero my review sample questionaires ako. nagbabasa din ako pero more on focus ako sa net surfing. minsan nag eexam din me sa internet questionaires then nakakakuha ako ng 60 pecent. i hope and pray sa board exam ganun din makuha natin or higit pa.:) God bless sa mga kasama ko na mag eexam. let’s just pray for it and xempre sabayan na din sa pag rereview:)

may nabasa ako. sabi before ka pa man din mag review, mag advance reading ka na, sa isang topic, mag basa ka for 3 hours, tapos 15 mins break, tapos 3 hours ulit, then next day ibang topic ulit, ganun din 3-15-3 everyday un. tapos kung di mo namn na kayang tapusin bago ka mag exam, ok lng, kc it’s the reality na hndi mo talaga kayang basahin lahat ng gusto mong mabasa, TRUST GOD ang last resort. Nursing Board Exam Test Taking Strategies
Introduction:
The board exam is a test for entry-level nurses. This means that it does NOT expect you to know everything. So, don’t panic when you encounter questions and you do not have any idea what is being asked. Use test taking strategies in answering it.
After doing an extensive review and you still feel like you don’t know enough, don’t feel bad. It’s natural to be anxious. A little anxiety will actually help you during the board exam as it will keep you alert and focused.
Test Taking Strategies:
Take time in reading the question, read the question carefully and find out what the question is asking. Keywords are very important. Keywords in the question are usually associated with the option containing the right answer.
Be aware of the time, if you don’t know the answer take a deep breath and proceed to the next question. You need to answer each question for around 1 minute. The remaining time will be spent on taking a break once in a while when you feel tired and shading your correct answers to your answer sheet. I personally recommend a 3 minutes break every 20 minutes of answering questions.
Never, change your answer unless you are absolutely sure with your new answer. (Erasures in answer sheets are not recommended)
When answering questions which you have no idea, choose the longest option. It takes more words to make the statement correct and also remember that more words may also make the statement wrong so read them carefully.
Always focus on the Patient’s Ability rather than the Patient’s Disability. Interventions must be client/patient centered.
In choosing answers, always begin with INDEPENDENT NURSING ACTIONS.
Nursing interventions:
Independent are nursing actions that the nurse may perform for the patient without the need of a doctor’s order.
Interdependent are nursing actions that collaborates with other members of the health team (Patient, Dietitian, Med-techs, Pharmacist etc.).
Dependent are nursing actions that requires a doctors order.
Example:
The nurse is caring for a patient with a fever. During the morning rounds, the patient complains of headache. The nurse proceeds to check the temperature and finds it to be elevated at 37.9 C, the nurse should:
a. Administer paracetamol to relieve headache and fever as ordered
b. Perform a tepid sponge bath
c. Document the temperature and call the Physician
d. Check CBC for signs of infection
In the example above, the independent nursing action is option B. Therefore option B is the correct answer.
Choose the option that is totally different from the other choices and stick with it.
Example:
The nurse is caring for a patient on heparin infusion. During the morning care, the patient complains of sore gums after brushing teeth. The nurse should:
a. Stop the infusion
b. Notify the doctor
c. Assess for bleeding
d. Administer protamine sulfate
In the example above, options A, B, and D are all nursing interventions. Only option C involves nursing assessment. Therefore option C is the correct answer.
If you don’t know the right answer, look for the wrong answer and try to eliminate it first. This will narrow down your options.
Another Test Taking Strategy is that if the question is asking for a positive answer, choose the answer with positive result and vice versa. Absolute words are usually negative. Choices with “always” “all” “never” “only” are considered negative. Nursing science in not absolute.
If you encounter options that are all correct, look for the umbrella type of option that fits all the other option. Remember that no two option can be correct at the same time.
One of my favorites among the Test Taking Strategy is that if you have no clue for any of the option, choose option C. Option C is usually the best or the worst answer that you could come up with. If you are the examiner, you always put more thinking of the option C making it a tricky option.
All of the Above option, and you are positively sure that two answers are correct, and one option is doubtful, choose the All of the Above option. Remember that no two option can be correct at the same time.
The Coma, Coma and Rule in Test Taking Strategy To use this, first categorize the options with the same answers, then look for the answer that is incorrect. All other option with the incorrect answer are wrong.
If the question asks about “What Nursing Action” Always think about the NURSING PROCESS “A.D.O.P.I.E.”
If the question asks about “Patient needs” Always think about the “MASLOW’S HIERARCHY OF NEEDS” and remember that PHYSIOLOGIC NEEDS is priority.
If the question asks you about “Prioritization” Always think about the (ABC) Airway, Breathing, and Circulation. If you cannot find these, choose the option which prioritize SAFETY.
In eliminating options, the similar options are automatically wrong options. For example bradycardia and slow pulse. Since both have the same meaning, automatically eliminate both of these option. No two option can be correct at the same time.
Look for opposite options. Example: High Blood Pressure and Low Blood Pressure. These options are the exact opposite. One cannot exhibit both symptom at the same time making one of the option correct.
In Psychiatric Nursing, verbalization of patient’s feelings is very important. Remember not to reinforce hallucinations and delusions. Acknowledge feelings and present reality. Nursing diagnosis is usually focused on Altered thought process/content. Goals set for patient/client is to have an optimal level of functioning.
In a community setting, the Vision Statement have words like disease free and progressive health for Filipinos. The Mission Statement have words like availability, accessibility, and affordability. The Goals Statement have words like promote, reduce, quality and equality of health for men and women.
Do practice in answering Nursing Board Exam Questions everyday as this will help you to develop test taking strategies. You need to answer at least 2000 Nursing board exam questions for you to pass the local board exam.
Other Test Taking Strategies in Taking the Nursing Board Exam
1. Read Questions Carefully
Scores on tests are greatly affected by reading ability. In answering a test item, you
should begin by carefully reading the stem and then asking yourself the following
questions:
• What is the question really asking?
• Are there any key words?
• What information relevant to answering this question is included in the stem?
• How would I ask this question in my own words?
• How would I answer this question in my own words?
After you have answered these questions, carefully read the options and then ask yourself the following questions:
• Is there an option that is similar to my answer?
• Is this option the best, most complete answer to the question?
Deal with the question as it is stated, without reading anything into it, or making
assumptions about it. Answer the question asked, not the one you would like to answer.
For simple recall items the self-questioning process usually will be completed quickly.
For more complex items the self-questioning process may take longer, but it should assist
you in clarifying the item and selecting the best response.
2. Identify Key Words
Certain key words in the stem, the options, or both should alert you to the need for
caution in choosing your answer. Because few things are absolute without exception, avoid selecting answers that include words such as always, never, all, every, only, must,
no, except, and none. Answers containing these key word are rarely correct because they
place special limitations and qualifications on potentially correct answers.
For example:
All of the following are services of the National Kidney Foundation except:
1. Public education programs
2. Research about kidney disease
3. Fund-raising affairs for research activities
4. Identification of potential transplant recipients
This stem contains two key words: all and except. They limit the correct answer choice to
the one option that does not represent a service of the National Kidney Foundation. When
except, not, or a phrase such as all but one of the following appears in the stem, the
inappropriate option is the correct answer—in this instance, option 4.
If the options in an item do not seem to make sense because more than one option is
correct, reread the question; you may have missed one of the key words in the stem. Also
be on guard when you see one of the key words in an option; it may limit the context in
which such an option would be correct.
3. Pay Attention to Specific Details
The well-written multiple-choice question is precisely stated, providing you with only the
information needed to make the question or problem clear and specific. Careful reading
of details in the stem can provide important clues to the correct option. For example:
A male client is told that he will no longer be able to ingest alcohol if he wants to
live. To effect a change in his behavior while he is in the hospital, the nurse should
attempt to:
1. Help the client set short-term dietary goals
2. Discuss his hopes and dreams for the future
3. Discuss the pathophysiology of the liver with him
4. Withhold approval until he agrees to stop drinking
The specific clause to effect a change in his behavior while he is in the hospital is critical.
Option 2 is not really related to his alcoholism. Option 3 may be part of educating the
alcoholic, but you would not expect a behavioral change observable in the hospital to
emerge from this discussion. Option 4 rejects the client as well as his behavior instead of
only his behavior. Option 1, the correct answer, could result in an observable behavioral
change while the client is hospitalized; for example, he could define ways to achieve
short-term goals relating to diet and alcohol while in the hospital.
4. Eliminate Clearly Wrong or Incorrect Answers
Eliminate clearly incorrect, inappropriate, and unlikely answers to the question asked in
the stem. By systematically eliminating distractors that are unlikely in the context of a
given question, you increase the probability of selecting the correct answer. Eliminating
obvious distractors also allows you more time to focus on the options that appear to be
potentially sound answers to the question. For example:
The four levels of cognitive ability are:
1. Assessing, analyzing, applying, evaluating
2. Knowledge, analysis, assessing, comprehension
3. Knowledge, comprehension, application, analysis
4. Medical-surgical nursing, obstetric nursing, psychiatric nursing
Option 1 contains both cognitive levels and nursing behaviors, thus eliminating it from
consideration. Option 4 is clearly inappropriate since the choices are all clinical areas.
Both options 2 and 3 contain levels of cognitive ability; however, option 2 includes
assessing, which is a nursing behavior. Therefore option 3 is correct. By reducing the
plausible options, you reduce the material to consider and increase the probability of
selecting the correct option.
5. Identify Similar Options
When an item contains two or more options that are similar in meaning, the successful
test taker knows that all are correct, in which case it is a poor question, or that none is
correct, which is more likely to be the case. The correct option usually will either include
all the similar options or exclude them entirely. For example:
When teaching newly diagnosed diabetic clients about their condition, it is
important for the nurse to focus on:
1. Dietary modifications
2. Use of sugar substitutes
3. Their present understanding of diabetes
4. Use of diabetic nutritional exchange lists
Options 1, 2, and 4 deal only with the diabetic diet, involving no other aspect of diabetic
teaching; it is impossible to select the most correct option because each represents
equally plausible, though limited, answers to the question. Option 3 is the best choice
because it is most complete and allows the other three options to be excluded.
As another example:
A child’s intelligence is influenced by:
1. A variety of factors
2. Socioeconomic factors
3. Heredity and environment
4. Environment and experience
The most correct answer is option 1. It includes the material covered by the other options,
eliminating the need for an impossible choice, since each of the other options is only
partially correct.
6. Identify Answer (Option) Components
When an answer contains two or more parts, you can reduce the number of potentially
correct answers by identifying one part as incorrect. For example:
After a cholecystectomy the postoperative diet is usually:
1. High fat, low calorie
2. High fat, low protein
3. Low fat, high calorie
4. Low fat, high protein
If you know, for instance, that the diet after a cholecystectomy is usually low or moderate
in fat, you can eliminate options 1 and 2 from consideration. If you know that the
cholecystectomy client usually is overweight, you can eliminate option 3 from
consideration. Therefore option 4 is correct.
7. Identify Specific Determiners
When the options of a test item contain words that are identical or similar to words in the
stem, the alert test taker recognizes the similarities as clues about the likely answer to the
question. The stem word that clues you to a similar word in the option or that limits
potential options is known as a specific determiner. For example:
The government agency responsible for administering the nursing practice act in
each state is the:
1. Board of regents
2. Board of nursing
3. State nurses’ association
4. State hospital association
Options 2 and 3 contain the closely related words nurse and nursing. The wordnursing,
used both in the stem and in option 2, is a clue to the correct answer.
8. Identify Words in the Options That Are Closely Associated With Words in the Stem
Be alert to words in the options that may be closely associated with but not identical to a
word or words in the stem. For example:
When a person develops symptoms of physical illness for which psychogenic
factors act as causative agents, the resulting illness is classified as:
1. Dissociative
2. Compensatory
3. Psychophysiologic
4. Reaction formation
Option 3 should strike you as a likely answer since it combines physical and psychologic
factors, like those referred to in the stem.
9. Watch for Grammatical Inconsistencies
If one or more of the options are not grammatically consistent with the stem, the alert test
taker usually can eliminate these distractors. The correct option must be consistent with
the form of the question. If the question demands a response in the singular, plural
options usually can be safely eliminated. When the stem is in the form of an incomplete
sentence, each option should complete the sentence in a grammatically correct way. For
example:
Communicating with a male client who is deaf will be facilitated by:
1. Use gestures
2. Speaking loudly
3. Find out if he has a hearing aid
4. Facing the client while speaking
Options 1 and 3 do not complete the sentence in a grammatically correct way and can
therefore be eliminated. Option 2 would be of no assistance with a deaf client, so option 4
is the correct answer.
10. Be Alert to Relevant Information From Earlier Questions
Occasionally, remembering information from one question may provide you with a clue
for answering a later question. For example:
A client has an intestinal tube inserted for treatment of intestinal obstruction.
Intestinal suction can result in excessive loss of:
1. Protein enzymes
2. Energy carbohydrates
3. Water and electrolytes
4. Vitamins and minerals
If you determined that the correct answer to this question was option 3, it may help you to
answer a later question. For example:
Critical assessment of a client with intestinal suction should include observation
for:
1. Edema
2. Nausea
3. Belching
4. Dehydration
The correct answer is option 4. If you knew that excessive loss of water and electrolytes
may lead to dehydration, you could have used the clue provided in the earlier question to
assist you in answering the later question.
11. Make Educated Guesses
When you are unsure about the correct answer to a question, it is better to make an
educated guess than not to answer the question. You generally can eliminate one or more
of the distractors by using partial knowledge and the methods just listed. The elimination
process increases your chances of selecting the correct option from those remaining.
Elimination of two distractors on a four-option multiple-choice item increases your
probability of selecting the correct answer from 25% to 50%.
More Tips:
• Cover the choices when reading the question. This way, your thought process will not be influenced by the choices.
• Before looking at the choices, try formulating your answer then look at the available choices. Choose the choice closest to your own answer.
• Always read all of the choices before choosing your final answer.
• Use the process of elimination. Write an X mark beside choices that are obviously wrong.
• Answer the easier questions first. However, mark the numbers that you skipped to make sure you don’t miss answering them later.
• Only change your answer when you are absolutely sure of your new answer.
• Between positive and negative answers, the positive choice is more likely the correct answer.
• The choice that is grammatically consistent with the question is more likely the correct answer.
• The answer containing the most information is more likely the correct answer.
• When “All of the Above” is included in the choices and you find two more correct choices, “All of the above” is more likely the correct answer.
• When you find two choices that completely negate each other, chances are one of them is the correct answer.
• If you find all choices to be correct, remember that you are looking for the best answer.

All of their advices may help you in preparation for the boards.. but of course without self discipline you will not succeed, Use your family as an inspiration, Reminisce all your hardships during the entire course of your college years, make your failure to lead you on the right path. is it still advisable na mag review center padin po? may inhouse po kc kmi pro parang nakukulangan din ako sa mga naituro nila.. nki sit in ako sa isang review center and ayun parang mas naintindihan ko dun. kaso naguguluhan pdin po ako kung itutuloy ko p yung pag lipat kasi baka hindi n maturo yung sa ibang topic n hindi ko npasukan sa review center na yun… tnx po

JUST ASSESS YOURSELF BEFORE TAKING THE EXAM. Are you ready? If ever you pass what is your plan? If ever you don’t pass are you still going to take the exam? Know also your capacity or physical condition before taking the exam. I graduated with honors in nursing, most of them, even the review center waiting my name to be on top because of all our tests I’m at top. Sad to say, I got sick due to super stress and expectation and still I insist to take the exam. I know all the questions but the problem is I answer it in a wrong nos. got mistakes and erase it all … you know what it mean? That is why never take exam seriously and do it privately, never announce that your taking the exams, less expectation, less stress and be sure you have a good health.

i didnt go to review center. its my 3rd time to take my NLE. I DID EVERYTHING BUT UNFORTUNATELY ITS NOT ENAF. EVERYTHING FALLS APART. SOBRANG TAAS NG PRESSURE KO TALAGA, ALL OF MY COUSINS AND TITA’S ARE NOW A REGISTERED NURSE AND THEY MAKE IT ONCE. ;(
