Intravenous Therapy (IVT) Training Accomplished Requirements form
This is the prescribed Intravenous Therapy (IVT) Training Accomplished Requirements form by the Association of Nursing Service Administrators of the Philippines (ANSAP) or the respective ANSAP-accredited hospital where the 3+3+2 cases should be written. It must be duly-signed by:
- the trainers/preceptors who witnessed the completion;
- and the chief nurse wherein the registered nurse has undergone the completion.

@nurse_Jay – ay sorry. di ko nalagyan ng label. yung una .pdf file then yung pangalawa, .doc (Word) file. Pareho lang sila, iba lang ang file type.

@nurse_Jay – long bond paper. you have to print your own form.
okay..ms.rona, one last question, pag isusulat na ba yung mga cases, hand written sya o dapat computerized din?
@nurse_Jay – doesn’t matter if it is hand-written.
miss rona do all we have to pay 1500 for completion of cases in ivt in martinez?

I’d just like to ask, what do you write on the IV Insertion column on the Blood transfusion part? Need reply asap.
@Angela – Blood type, volume and components (PRBC, FFP, etc.)
miss Rona, parang may mali sa form.. dun sa number II and III.. instead na type of cannula, dapat yata diagnosis yun nakalagay? nakita ko lang po sa sample of filled out ivt form.. =]
@harry – that sample of filled-out form is from the 7th edition of the ANSAP Manual (green book). You can just follow what’s in here.

Hi, the information on the above accomplished forms, I think are misleading…can somebody upload the updated version and erase this one?
@mikeru – In this situation, I think the better or appropriate term to use is “incorrect” and not “misleading” because for obvious reasons, I have no intention of giving or sharing false information. Thank you for the heads-up. Do you happen to know what the updated version has in it? If you do, do you have a copy that you can send to me? The type of IV fluid such as Lactated Ringer’s solution, 0.9% NaCl (normal saline), 0.25% NaCl, etc.
