PRC Initial Registration Instructions for December 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2011 – 2014.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)

@joanna – that schedule given by PRC is just for crowd control. you can register anytime after THAT schedule. Usually sa PRC na ang registration kapag late registration.
Now ko lang nalaman na sa QC na registration. May metered stamps nba dun and notary if ever. Pls reply, my sched is tom. thanks

@iraem – kung wala kang gala uniform at wala ka ng pambili or pampatahi.
Ngayon ko lang din nalaman yung tungkol sa registration, so just to clear, the sched is just for crowd control, however, after the sched, we can register anytime? That won’t affect anything?

@gee rn – I assume they are all just the same in all PRC branches. When in doubt, call PRC themselves to inquire. You may also ask your local PRC office on what you should pay for first — usually though, it is oath-taking ceremony, PRC then PNA.

miss rona hindi ko po natapos ang pagregister kanina sa PRC manila. nasa akin po ung oath taking form. notarized and with metered stamp na po. saan ko po kukunin ang NAF and PERRC? s prc pa rin po ba or sa osahc na po? ang nakalagay po kasi sa step (3) ay pumunta s window 14, 19, 28. sabi po ng guard ay hindi daw po doon sa PRC un.
@angelique – did they ask for the NAF or PERRC? And yes, consequently, since hindi sa PRC ang venue, wala ng windows na susundin. Ano ang sinabi sa iyo sa registration? Balik ka bukas para tapusin ang pag-rehistro?
naku.. bakit naman gnun? ang gulo ng proseso nila dapat tulad ng procedure sa prc ang ginawa nila un ung alam ng mga magpaparegister.. haaayyy.. bukas pa naman ang sched ko dun sa OSHC tomorrow… haaayyyzzz sana di hassle. ano ba ung NAF at PEERC na yan? wala naman un sa requirements na hinihingi ng prc eh… kainis paggan2ng ang layo layo ko sa manila.. haaayy..
relax ka lang. huwag ka masyado magpa-stress. ang bilis mo naman magulantang eh registration lang naman ‘yan. so far walang nagrereklamo sa registration sa OSHC. malamang sobrang madali lang at hindi mahaba ang pila. Ibang-iba kung sa PRC ang registration. mas pinadali na nila at wala ng windows na pipilahan dahil redundant ang steps kung sa PRC mismo ang registration.
Mam rona ask ko lang po kung pwedeng hyphenated name ang gamitin ko for initial registration kahit di yun yung ginamit ko nung exam. Btw, married na po kasi ako and i want to use the hyphenated for my license. Eh ung surname po kasi ng hubby ang ginamit ko nung exam. Thanks po!
@tonet – Better bring your documents (marriage certificate, birth certificate, etc.) when you register and inquire from PRC themselves what you should use.

@Lanie – usually, you are supposed to register where you filed to take the board exam since your documents are in that PRC office.
Instead of getting the license itself, is it possible na PRC na magpadala (like passport delivery)? do they offer such service?
@JKTG – itself? did you mean myself? as of this writing, PRC doesn’t send PRC licenses through courier or mail.

mam clear ko lng po kc mejo nalilito ako.. late registrant po kc ako, nakalagay po dun sa press release ng initial registration schedule na march 10 sa PRC n po, pero meron po isang post na cnabi nio po sa agham parin.. ok lng po ba sumabay ang late registrant sa mga nakasched dun by that day?
@bhieanne – what press release are you referring to? according to the schedule released by PRC, late registrants can register on the 10th at the OSHC. @Alzen – Usually, you will have to register in the PRC office where you filed to take the board exam cause that is where your documents are. You can take your oath anywhere you want to.

I have yet another complaint that needs to be addressed. This one concerning the Professional Regulation Commission (PRC) of the Philippines and the Board of Nursing (BON) of the Philippines.
Why do both of these organizations seem to keep us student nurses and nurses alike in the stone age? Both websites are terribly out-dated.
The PRC does not have a search engine on their site and they lack any valuable forms that may help university students, of different courses, to file the cases that need to be reviewed by the organization. Instead they rely on each school to properly disseminate the “up-to-date” forms to their students. I asked for a copy of the OR case form from my Dean’s office. It turns out that the copy is currently not the one being used!
In regards to the BON. It’s not only their lack of a proper website, which has no helpful navigational tools, but also their policies that keep the Philippines behind in health care, especially nursing. Why do they continue to force nursing students and instructors to wear ridiculous all white uniforms with white shoes? Are they not aware of something called “white-coat hypertension”? How many Filipinos experience this condition because of this out-of-date policy? And the shoes are dress shoes. A few months ago an instructor slipped on spilt blood in the ER and fell, breaking her hip. All thanks to a policy set by the BON.
I hope these things stop going unchallenged and unchecked here in our great nation.
