PRC Initial Registration Instructions for December 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2011 – 2014.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)













hi RN Rona… I took exam sa Iloilo City, Dec. 2010…and thank God I passed …but now Im working in one of the private company here in Laguna… pwede ba na I will register sa PRC dito sa Manila? and take Oath also here? thank you…
@HONEY rn – you can take an oath anywhere you want to but you are supposed to register where you filed to take the board exam.
ms., sa iloilo po ako nag file for board exam and i already passed. is there no chance na dito ako sa manila mag register for my license?
@steph – ms., usually, you are supposed to register where you filed to take the board exam because your documents are in the PRC office.
I took the exam at Iloilo and I’m also applying for my license at Iloilo this week but I will be taking my oath in Manila next week.. what is the process for my oathform? can i submit it to PRC Iloilo without a signature from the person administering the oath to us?? thank you so much.
@nurse101 – yes you may. The oath form won’t be signed during the oath-taking ceremony at the SMX.
Mam Rona RN: one of the requirement for the initial registration is the 2 sets of metered documentary stamps.. sa prc lang po ba un mabibili?

and I also want to ask narin po regarding the dress code for male on the oathtaking ceremony.. Dito sa site niyo, you said that it’s white duty uniform, and sa PNA site naman gala uniform? which is which? Thanks! GBU

Hello MS ROna i would like just to ask you about masteral what is the difference between MSN and MAN and if i choose one of them which i would benefit for atmost i am just recently pass the december 2010 board and also if you dont mind where could i get a training about IVT thanks
@ELSON DUCO RN – I honestly have no idea what’s the difference between MSN and MAN let alone which one will benefit you most (for whatever purpose that I assume you wish you would). For IVT training, check in the right side of the page the monthly compilation of training and seminars.

@RianaRN – what certificate are you referring to? you do not need to GET any certificate to be able to register for a license at the PRC. You are actually RECEIVING one after registration.

I just want to ask if, possible po ba na bumili na ng metered stamp beforehand sa PRC para po maging mabilis ang registration procedure or we have to by it on the same day upon registration at exclusive lng po na sa venue dapat bumili?

@Julliean – pwedeng i-download. Walang instructions sa taas na kailangan na ipa-notarize siya bago pumunta sa PRC.

when i took the exam last dec the name that i used was my single name and now that i passed i wanted to change it and use my married name to appear on my license. would that b possible?
@pam – I am not sure if that is possible. you will have to inquire about this during your initial registration. bring all the necessary documents such as your marriage certificate and also your birth certificate too.

@misayrn – usually, you will have to register in the PRC office where you filed to take the board exam cause that is where your documents are.

@jet – yes of course. walang bisa ang sinumpaan mo kung hindi naman pinirmahan ng isang Notary Public.
Ma’am pwede din po ba mgparegister sa prc ever gotesco sa recto if late registrant?
@jaja – as of this writing, I am not certain if PRC Ever Gotesco can process registration.

meron po akong maliit at (makulit) na problema. I have long hair (shoulder length) and ayoko sana magpagupit para sa oath taking. plano ko sya ipa braid as in walang lalawit na hair. do you think its acceptable? thanks! ^^
@Mark – kulit nga ng problema mo haha. I am not in the position to say if it’s acceptable or not cause I am not affiliated in any way with BON or PRC but the requirement did say “hair not touching the collar” and technically, your hair will not touch your collar kung naka-braid. In short, it really is up to what will happen on the day of the mass oath-taking ceremony. Best of luck to you!
thanks mam rona sa reply mo about my “makulit” na problem. nakapag decide nko na hindi magpapa gupit para sa oath taking bahala na si batman. basta ipapa braid ko na lng to ng bongang bonga as in kapit sa ulo walang lalawit. pag di ako pinapasok sa SMX magwawala ako dun! hahahaha.. ^^ (dun sa mga me tanong try nyo munang magbasa nung mga post kasi malamang yung tanong nyo eh nasagot na)

Hi Mam Rona, I recently passed the board exam and I’ve been reading your posts and instructions. I made a mistake though, when I went to PNA to get my oath taking ticket, I didn’t know the supposed schedule of our school so when the facilitator there asked me the date, I told him March 14, pm. Then just yesterday I found out from a friend that I should have looked at the schedule of our school. WILL THAT BE ALRIGHT, OR IS IT POSSIBLE TO CHANGE THE TICKETS TO THE RIGHT DATE?
@Faye – it’s all good. as for changing the tickets, you may call PNA if they would allow an exchange if you still want to join your school’s schedule.

@chel – no. naturally, you will have to ignore that since the venue is now at OSHC.
