PRC Initial Registration Instructions for December 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2011 – 2014.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)

You can ask around inside PRC where you should go to register.

hi mam rona! ask ko lng po..magpaparegister na po ako bukas sa PRC manila pede na po ba? tsaka ung pong gagamitin kong cedula ay yung na issue pa po sa akin last august 2010, pwede po ba un? o dapat po kumuha na ako ng bagong cedula?
@lou – call PRC first to inquire if you can register tomorrow. Just to be sure so you do not waste any time, effort and money. The cedula should be current so you’d be getting a new one (year 2011).

Last document i need para matanggap ng work is ung cert of registration… Eh,Nawala ng prc iloilo ung registration certificate ko! Nwalan ako ng oppurtunity pAra mag trabaho dahil doon! 2007 ako pumasa ng nle hanggang ngayon 2011 wala pa ung certificate of registration ko!!! So ulit naman ung ielts n processing fee, ulit naman bayad ulmg 40,000 php plus!!! Mga walang kwenta!!!! Tangap ng tangap ng bayad hindi naman “professional” ang prc!!!!

I would like to ask qng kelan po ba ire-release yong license namin almost 4months na po kaming nag process,,bakit naman po masyadong mabagal.nasasayang po tuloy yong mga oppurtunities na dumadating dahil sa wala pa kaming license.Sana tuparin nyo naman po yong sinabi nyo ng oathtaking na mapapabilis nyo nagun ang pamimigay ng license at boardcertificate,Plzzz….

hi to all! panu po kung nakapag oath ako pero hindi ako nagkapagparegister after ng oath kasi umalis na ko para mag abroad.. kasi maeexpire na yung visa ko.. panu po iyon?
july 2010 po ako nagtake ng exam.. tapos nag oath ako ng september pagtapos.. wala na kong ibang procedure na ginawa.. hindi na ko nakapagparegister kasi umalis na ko the following month..thanku..

mam ok lang hu ba iba ung magregister sakin? pero bigyan ko nalang ng letter na pinahintulutan ko sya mag registro sakin….kac nakapagregister po ako sa manila kaso sa cebu ako nakapasa hindi napo ako nakapasok sa verification pero napirmahan napo ung registration form ko ng attorny pagverify nalang pede hu bang iba na ung papuntahin ko sa prc ipapadala ko nalang sa iba ung registration form ko eh magbabayad nalang ung ko lang ko….pls answer
Better ask PRC themselves. I am not sure if they will allow a representative to accomplish the registration process for you.
Good day! Sabay ba marelease ang ID & certificate? ksi walang date of release nakalagay sa claim slip ko sa ID, yet sa certificate meron. Thanks!
Usually sabay na binibigay ang certificate and license card.

hi good day. i need help po i went to PRC davao yesterday to register i took NLE and passed on March kaso ngayon pa ako magparegister and they require me to present a letter from the BON stating the reason why ngayon lang ako nakapagregister. is it necessary to have that letter? if yes do have sample or do you know any website that i can help me. thanks po in advance… pls email me po pag may sample kayo thanks..

Hello po ma’am rona..im a december 2010 board passer. nang makuha ko po yung prc lic ko po,hindi po kasama na naibigay saking yung certificate of registration ko po. im wondring, pwede ko pa po kaya makuha yung certificate of registraion ko po kung pupunta ako PRC? I applied for RN Licensure by Endorsement in TEXAS BON . Even if I only practiced 2 yrs. in Phil.( 1964-1969 ) and over 35 years here in USA , took state boards and got my BSN degree in one of USA universities , they still need RN License Verification from Phil. BON . How can I get this verification if PRC Phil. BON lost my file ? Don’t they keep copies of board examinees and their scores somewhere ? Unfortunately , I don’t remember my Phil. RN license number.
