PRC Initial Registration Instructions for July 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2010 – 2013.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)

ma’am rona i badly need your help…just wanna ask poh…kasi as far as i knw nah hindi pwede makapag reg if wasn’t able to take an oath…pano poh yan eh Oct 1 poh ung oath taking dito sa bacolod and nakapag process na poh ako…what keeps bothering me is that ung sched koh po for regstration ds sept 28 sa V-tech tower…so im planning nah punta jan sa manila then be back for my oath taking…pwede lang poh ba yan nag mag pa reg even di pah koh naka pah oath?..sorry for the dstrbanzh!..and thank you in advanzh!…^~^…

I got my oath tickets na po pla. We were @ PNA, 3am tapos were nos. 700 and up! tapos were asked to come back 1pm pa. But once ur inside PNA mabilis na po…
We are given blue colored nos. tapos meron pa nxt batch they were given red colored ones. i guess the red colored are those na hindi umabot sa 1200 na cut off and they are just considered since they’re already there. about 500+ pa yung isang nakita ko na may red no.
@abee – thanks for sharing that information. this would help those who are still in disbelief that there’s a line as early as 3 AM.
one more thing, they asked for my board rating po or a newspaper…i guess that is they’re way of validating that you really passed the board exam.
Hi Rona, i filed for certicate of registration last week and will get it on the 22nd. my question is, if we get the certifcate, will it already include our license number? i’m asking coz i’m applying for NCLEX and it requires the license number. if it doesn’t include it, maybe i can just write them why my license hasn’t been issued yet and give a copy of the certificate you think? thanks!
@boomboomRN – Fortunately for you, the answer is YES. The Certificate of Registration will have your license number on it. This is what you would see there:

@ann – can’t assure that you will be able to register anywhere else aside from where you filed to take the board exam. you can take your oath anywhere you want to.
@mam Rona,, ask ko lang po kung ung Certificate of Registration po ba ung bnabayaran namen ng 75php per copy?nag fill up kasi kme sa PRC 1 time, request a copy of Certificate of Board Rating and Certificate of Board Passing
@jek – No. Yung Certificate of Registration, kasama na yun sa babayaran na Initial Regsitration fee na PhP 600. Yung PhP 75 na binayad niyo, iba pa yun. Para yun sa ibang certificates (like nga yung Certificate of Board Rating or Certificate of Board Passing).
@jek – tatlo (3) kasi yung certificates na pwede mo kunin sa PRC.
1. Certificate of Registration (ito yung parang “diploma” mo sa pagiging RN. Payment for this has been included in the initial registration fee of PhP 600)
2. Certificate of Board Rating (ito yung grades na nakuha mo sa 5 areas of the exam; may bayad ng PhP 75)
3. Certificate of Passing (nakalagay lang dito na pumasa ka and kung ano yung total average mo sa exam; may bayad ng PhP 75)

do we need to bring the NOA during our registration? i believe we need 2 copies of 2×2 pictures with us, i have one left, and i dont wana spend anymore, can i just take the picture from the NOA instead? hehe thanks!
@2by2boy – a simple answer to the simple boy… I’m just a girl, standing in front of a boy, asking him to… do the same thing that I did when I registered in the PRC. That’s exactly what I did so that I don’t have to get another picture taken.
You do not need the NOA. You get to keep it with you. Remembrance of that day when you were taking the exam.

hey good afternoon.. i just went to PRC yesterday to apply for my certificates, yung guard po dun sa PRC ayaw magpapasok kung hindi mo dala yung NOA or claim stub, good thing i came prepared. i just want to share that lmao kasi medyo masungit na yung guard kahapon. ask ko lang po kung pwede sa ever recto ako magparegister kasi schedule ko na po bukas..baka po kasi mahaba pila sa PRC sa dami ng tao and if kung may documentary stamp na din po ba dun

yeah this is what I was telling the others about before. That PRC holds oath-taking ceremonies but it looks like not in their small offices now. Parang madami pa kasi na gusto din ng ceremony kaya sa auditorium nila gagawin.

Your registration number will be your license number.

@boomboomRN – basta nakatulong sa’yo okay na yun. Luka ka! Hahahaha. That 86,000 thing is the serial number (SN) of the documentary stamp. The Registration Number/License Number is in this line:
“Sa Katunayan Nito, Itong Katibayan Blg. (your license number) na nilagdaan ng blah blah blah…”
Binura ko lang yung nandoon sa document kasi siyempre, baka may magpagawa sa Recto. Alam mo na. Haha.

Hindi na yan katulad ng tickets sa PNA. Just make sure you don’t go there after lunch. May nag-post kasi dito na 3 PM pa lang umalis na ang tao ng PRC sa V-Tech. Di sila tuloy nakapag register.
a naughty question from a naughty boy…umm… sa oathtaking in PRC, it was mentioned here that it’s 9am-12pm..strictly dont be late ba yun? hehe and no need to call PRC that i will attend a specific date w/c is 22nd or 23rd….what if i got there on the 22nd, sigurado kayang they will be able to accomodate all of us…baka kasi i have to notify PRC pa that Mr. 2by2boy will go on the 22nd….watchuthink?

What I know at the moment are based on comments and information given to me by those who have called or asked the PRC. Let’s assume until there’s no other notice that you just have to go there and attend their oath-taking ceremony. Of course, that could mean that it will be one big circus if they wouldn’t make a little effort to make their oath-taking ceremony some semblance of organization. @kikay – there’s no registration form to fill-out. The Certificate of Registration won’t be available right away.

@kikay – It is not called a Certificate of Registration for nothing. Why would the PRC ask for a Certificate of Registration from you during your registration? They would give it to you AFTER you have registered.
ask ko lang ma’am rona..kakapasa ko lang po this july2010 tinanggal nila ako kasi 19years old palang ako.. nagstart ako magvolunteer nurse sa isang secondary hospital Sept 1, 2010 ako ngstart but last tuesday, they informed me na baka i terminate ako kasi may order daw ung “ANSAP” na bwal mag volunteer ang 20 years old and below.. hinahanap ko po ung legal basis nito dito sa net pero wala akong makita.. memron ba tlgang rule na ganon?

i am kinda lost.. i failed to register sa pna last sept 16 bec out of tickets na daw nung 15. my parents would want me to take oath in roxas.. should i process it here then or should i just authorize anyone there in my province to take care of it?
@mau – I don’t quite understand. You got me lost too. Register in PNA? Did you mean register at the PRC? Or you simply just weren’t able TO BUY tickets at the PNA? And authorize somebody to process WHAT? Your registration at the PRC?
i mean, to get tickets pala from pna.. and also, for my registration sa prc, sa sampaloc ko lang din po ba ipprocess?
@mau – tickets for the oath-taking ceremony are already sold out. tagal na. you will register in PRC Sampaloc yes. that is if you are scheduled to register there.
ma’am ask ko lang po. pano ung mga nmove sa registration? ngayon ko lng po kc nalaman, nmove po pla ako sa sept. 16. pwede po ba humabol?
@GUEST-J – you are not the only one who is in the same situation. Dami din na huli na nila nalaman na may bago na naman palang schedule. I am afraid you will have to register in the date for the late registrants. You can go to V-Tech and try if you really want to, though. Or better yet, call PRC first to verify if you can make habol. Their numbers are on the right side of this page.

@rony – I will look in to this. I am not aware if there are any memorandum for the age that one can volunteer in a hospital. “Volunteering” itself is questionable kasi. Let me know too if there are new development in your case.
Hi Mam Rona. ung sister ko po pla, tumuloy pa rin sa manila. oath taking daw niya sa SMX today ata or bukas. Ang tanong ko po, required ba ung ticket? I’m wondering kung saan nakakuha ng ticket ung scammer pra ibigay sa ate ko. hindi ko pa natawagan ang PRC to verify name ng sister ko kasi I don’t know if someone will entertain me ba w/out her examinee number. tska, long distance call din so I make sure na hindi sayang ung call ko if in case. For sure nabasa nya ung comments natin dito pero she ignored it pa rin.
@Gladys – attending a mass oath-taking ceremony is not a requirement e.g. the one in SMX. I do not know how this person dealing with your sister was able to produce a ticket for the SMX oath-taking ceremony. I really hope she would think twice before really going with this. well yes, i havent registered sa prc yet miss rona, and hindi ako nakakuha ng tickets sa pna.. then i was asked by my parents nlang po to take oath sa roxas.. im currently in rizal so i dont see it possible for me to have myself get the tickets personally..
im asking kung pwede ko po kaya ayusin yun lahat, that of my registration, and the oath taking sa sampaloc..
and in any case that it wont be possible for me to take care of my oath sa roxas, what would be po the possible things that i can do?go authorize someone to arrange everything for the oath taking sa roxas?

Ayaw talaga ako pakingggan Mam Rona. Kasi ung tumawag sakanya ay sinabi kay ate ko na kunin na lang nya ung license niya sa PRC daw mismo. May claim stub pa na binigay sakanya. Napaisip ako, hindi kaya ung tumawag sakanya ay baka nilagay na niya mismo dun sa claim window ng PRC? so parang magiging positive nga na possible na may empleyado ngang gumagawa ng anomaly? Wala ba kayong kakilala sa mismong PRC na pwde natin ipaverify ung name ng sister ko? pero hindi ko alam kung anong pangalan nung tumawag sakanya. Ayaw kasing sabihin ng ate ko eh.

Yes you can. Signing and submitting a notarized oath form is oath-taking.
You can ask somebody to buy tickets for your oath-taking ceremony in Roxas. You do not need to buy it personally.

Mam Rona, my sister texted me thru my roaming number. na sakanya na daw ung certificate of passing certified true copy daw na sakanya and then PRC will mail her certificate of passing as well on the school she graduated. and ipapadala na sakanya ung claim stub this week. I’m confused if totoo nga ba talaga na pumasa ung ate ko even w/out her name sa PRC website.
Mam Rona. Nakatawag na ako sa record section ng PRC. sabi, personal ang pag verify ng name. What I did, sinabi ko ung concern sa ate ko and that girl told me, nabiktima ang ate ko ng fixer. I will send someone (a relative) to verify her name sa window H (record section) She even asked kung anung name nung kacontact ng ate ko I told her na ayaw sabihin ng ate ko ung name.

Yes it’s possible but we’ll never know for sure until it has been proven that someone working at the PRC is involved in this activity.
I do not know anyone working at the PRC. If your sister won’t say the name who called her, it could be that she was asked not to to avoid getting caught.
What I know is that PRC doesn’t send a certificate of some sort to the school where a nursing board exam applicant has graduated from. If a claim stub will be sent to her, that means that she is already registered without her going to the PRC not even once.
Please don’t be confused. I have said this to you before and I will say it again. The only list that is official is the one released by the PRC in their website and to the newspapers with a wide circulation in the Philippines. If your sister’s name is not on that list, she did not pass. End of story. She is involved in a suspicious activity.
Enough said. Now that you’ve told somebody at the PRC of what’s going on, I hope they will do something about it. However, you have called my attention on this thing many times and I am forced to make some action. You are not the only one who is asking me about this. There are a couple more who has a similar situation . I will make a post out of this comments and we’ll see what happens from there.
Mam Rona, I need your help. I’m protecting my sister about this. Dapat mahuli kung sino man ung gumagawa ng anomaly. Andito kasi ako sa states so I can’t verify her name personally sa PRC. I called my dad baka siya ang papupuntahin ko to vefify my sister’s name. I keep bugging my mom to ask my sister kung anong pangalan nung tumawag sakanya. Kinukuha ung name nung tumawag sa ate ko nung nakatawag me kahapon sa PRC record section pero I can’t give it kasi hindi naman sabihin ng ate ko kung anung pangalan nung tumawag sakanya. Sana makicooperate ate ko about this para malaman kung sino ba talaga iyong tao na nakausap niya. Nakapag-oath taking pa ang ate ko nung lunes. nagtataka ako kung paano siya nabigyan ng ticket.

This is the most that I can do at the moment. I am hoping that somebody who can investigate on this will read that post. All your comments and my replies are in there. I will just edit it when you have a new comment. Your sister is obviously not cooperating with you. Either she’s really willing to go through this or she was told by the person she dealt with not to tell his/her name. I am also wondering how she was able to buy an oath-taking ceremony ticket at the PNA. The following things were asked by the PNA during ticket-selling:
-ID
-sequence number of the board passer in PRC’s list
-show a verification slip from the PRC that shows that he/she passed
-newspaper with the official list of the July 2010 nursing board exam successful examinees
ma’am Rona! question, i applied for board rating in PRC few weeks ago and was supposed to get it yesterday (Sept.22) but I wasn’t able to go there. You think I can claim the board rating in V-tech tower when I’ll be doing my initial registration on the 30th which is next week. thanks
@boomboomRN – hmm. I don’t think so. I wouldn’t rely on the PRC representatives who would go to V-Tech that they would seek out and bring with them your made Certificate of Board Rating. It’s best to verify with them first though. That is if you can make them pick-up their phones, he he.

Ms.Rona, I hope you could help me with this. I took the board exam with Santos as my surname. I got married just before the results came out. Now Im having a dilemma whether which surname should I use to register for my license. Is it Santos still or should I use my hubby’s surname? If it’s my hubby’s, what additional requirements should I bring? I’m quite confused.. THANKS!
@Chloe – This is PRC’s Policy on petitions for change of name, status and birthdate and/or correction of name: http://www.philippinenursingdirectory.com/prc-resolution-no-2003-136-series-of-2003/. That resolution is dated 2003 though. Please call PRC to verify if there’s no amendment to this.
