PRC Initial Registration Instructions for July 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2010 – 2013.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)

Although I am concerned with the certificate indicating I am a board exam passer. I was told by the records department of PRC that they are not processing this kind of document until after 2 weeks.
Sa manila po ba lahat mag oath taking?
@jericho – may mga training centers, di naman nila nire-require kaagad na may PRC license ka na. Your claim stub will do. There are also some training that does not require a PRC license such as BLC and CPR. Kung sa Baguio ka nag-apply for board exam, doon ka din dapat kumuha ng lisensya mo. Kung saan PRC office or kung ano ang PRC office ka kumuha ng board exam, doon ka dapat mag initial registration.
@matt – yes alphabetical ang schedule ng pagpunta sa PRC para sa initial registration. Usually 1 month lang naman mula hanggang A to Z. Kung T ang first letter ng surname mo, malamang mid-November ka pa makakapag-register.

Yes di ka pwede pumunta sa PRC kapag hindi pa araw ng regsitration mo. Relax-relax na. Makakapg-register ka din. You’ll definitely get your PRC license this year.
@TORDY – no you do not need to register first to the PRC before you can buy your oathtaking tickets. You can go ahead and buy your tickets now. Registration to the PRC will presumably start on October pa.
@jan – Hindi pa nagstart ang initital registration sa Manila. Sa October pa yun. I will post the schedule when I get a copy from the PRC. Hindi pa pwede magparegister kung hindi mo pa schedule ng pagpaparegsiter. Yung oath form, pwede na ikaw na ang magprint para pagdating mo sa PRC, hindi ka na doon mag fill-out. Don’t worry with the questions. I am here to help and answer questions as much as I accurately can.
@Jasmin – No unfortunately you can not register ahead of the scheduled initial regsitration dates. Unless you can pull string sa PRC to let you register earlier than supposed to, the option for you is this: Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.
@Ervin – yes you may ask for envelopes at the PRC. I just do not know if you have to pay for it.
@Darwin – no hindi lahat sa Manila dapat mag-oathtaking. Pwede ang mga taga ibang regions mag oathtaking sa Manila though.

..hi po?, ask ko lng if ok lang na sa PRC manila n ako mag pa register and kumuha ng license even though I took the NLE on Baguio?,, then now po n I passed the exam… anu2 n po b ang kelangan ko gawen?, ung ticket po b sa oath taking?, where can I buy it…. Sobrang thank you po… =)
@camz – you can’t register and get your license in PRC Manila if you took the NLE in Baguio. Kung anong PRC office ang nagbigay sa iyo ng board exam, doon ka dapat magpa-register and kumuha ng lisensya. Now that you have passed the board exam, just wait until i can get a hold of the PRC-BON’s schedule of the regional oathtaking. You can attend the oathtaking at SMX Mall of Asia if you want to, though.
@Darwin -wala pa ang schedule ng regional oathtaking. As far as I can see in the BON website, wala pa sila pino-post.

@bhel – No. PRC initial registration would definitely be scheduled after the mass oathtaking on September 20 and 21. And it would be in alphabetical order so you will have to follow the date that you will have to register depending on the first letter of your surname.

@matt – walang dapat puntahan sa PRC sa ngayon. You won’t be entertained as long as it is not your day of initial registration yet. Most probably sa October pa ang start ng initial registration.

@veronica – sa NCR ba ang school mo? If it is, you can call PNA at 521-0937. You can ask for your school’s schedule (date and time) of oathtaking.

@matt – Yes only the PNA has the tickets. Doon mo lang mabibili. You may check with your school first (if your school is in the NCR or nearby province) if they already have gotten hold of it though. Minsan kasi binibigay na ng PNA ang tickets sa schools para sila na ang magbenta sa board exam passers/students nila.

Ask your friends who took the board exam in Pampanga as to where the PRC office is. Pwede naman na doon na siya magboard exam din sa December 2010 kung mas gusto niya doon. Usually sa provinces kasi 2 days lang or 1 week ang binibigay na time para makapagfile to take the board since kaunti lang naman ang volume ng test takers in the area.

@andi – Yes, kapag nag-apply ka ng 1-year membership sa PNA ngayong September 2010, expired na siya sa December 2010. Hindi ko alam kung bakit ganun ang patakaran nila. In effect, magbabayad ka ng 1-year membership fee pero 4 months lang ang validity ng membership mo. Ang start ng “official” membership date para masulit mo ang binayad mo for a year is every January.

Rona, one of the requirements for registrants outside the country is this:
Certificate from the PRC indicating that the registrant had passed the Board/Professional Examination (present original and submit photocopy)
How would I be able to present them an original if I’m in another country?
@Ervin – Remember when you applied for the board exam:
“STEP 4: Pay for the following at the CSC: metered documentary stamps, and; mailing envelope with metered documentary stamps”
After the release of the board exam result, the PRC will send a snail mail of your board exam result to the address that you indicated during application. You can have it sent to you when it arrives in your address here in the Philippines.

Hi! pano yun kung mag a-apply ako for nclex they need some sort of certificate that I passed the NLE. Can they give me some sort of document that will support this? something that I don’t have to wait for for two months? Thanks!

Ma’am Rona, ung Certificate of passing same po yun sa PRC Certificate tama? so 2 months after the intial registration?.
ung grades po kelan pwede mkuha at pwede po ba na pati rank? if ever nasa top 20 po kasi may iba nagsasabi nsa top 20 sila.

Technicality part lang ang initial registration sa PRC.
1. Yes makakapag oath taking ka kahit hindi pa nakakapagregister. That is the actual process. Oathtaking FIRST then REGISTRATION.
2. Ang PNA membership ay para… (i’ll just get back to you on this one) Advantage of being a member of the PNA ay may discount ka sa mga seminars nila. That’s the main advantage that I can think of off the top of my head. (i’ll let you know of the others soon, hehe)
3. Yes, pwede ka na mag-apply kahit di ka pa nakapag-oath or register. Pwede mo naman sabihin na nakaschedule ka na for those activities. If I were you, I won’t be too worried too much if I can or can not put “R.N.” in your resume. Just put it cause practically speaking, you already are an R.N.
@beginner – yes, ang Certificate of passing is the same as PRC Certificate. PRC’s length of processing of their different certificates varies kasi. Minsan matagal, minsan mabilis. Depende sa volume ng kailangan gawin.

Yung grades and rank nakukuha din yun. You can apply for it during your initial registration. They will give you a specified date when you can get it from the PRC.

ang mahal po kasi ng registration then 4months ko lang magagamit, kung hindi rin po pala ko aattend sa mga training and seminars okay lang kahit anytime ako magparegister?.. (basta siguro january para sulit

@Erian – you are correct. Kung hindi ka pa naman aattend sa mga seminars nila, i don’t see any other need for you to be a member of the PNA. Tsaka sayang ang 1-year membership kung mage-expire naman sa December 2010. And korek, sa January ka na magpa-member para sulit, hehe.

@Che – yes ticket lang ang requirement for the oathtaking. The PRC “annual” registration can be indeed called or referred to as renewal of license. Hindi dapat annual ang tawag doon dahil nga 2010 – 2013 ang validity naman. Di ko lang alam kung bakit “annual” ang tawag ng PRC doon, hehe.

1. Sa PRC Manila ka dapat mag initial registration kung sa Pampanga ka nag board exam.
2. Walang PRC Regional Office sa Pampanga.
3. Wala pang schedule na ni-re-release ang BON for regional oathaking pero pwede ka umattend sa mass oathtaking sa SMX Convention Center.
4. Yes pwede kang mag oathtaking sa PRC. Sabihin mo lang na di ka pa nakakpagoathtaking. Sila ang magschedule ng oathtaking mo.
5. Walang service ang PRC para magdeliver ng PRC ID. Kailangan ikaw ang kumuha nun.
6. Ang PNA membership ay nage-expire every December. Kaya kung magpapamember ka ngayon, expired na siya sa December.

1. Sabi mo pwede mag-oathtaking kahit di pa register (OCTOBER 20 yung sched ng initial registration date ko)
2. If ganun, bakit may OATH form pa eh, para yatang appropriate yun bago ka mag-oath? Where in fact yun eh kasama dun sa instructions nila sa initial registration. Mag-ooath muna ako bago magpasa nung form na yun kasi Sept yung event tapos registration sched ko Oct 20?
3. Sa madaling sabihin ba, eh ang talagang aasikasuhin ko lang muna eh yung pagbili ng tickets para sa event sa SMX?
4. Wala na ba akong issubmit muna bago pumunta doon? so sa mismong PNA magvavalidate na umattend ako dahil bumili nga ako ng ticket nila?

1. Yah dapat lang naman na mag oathtaking ka bago magregister.
2. May oath form dahil yun ang pipirmahan mo at isu-submit sa PRC kapag nagreregister ka na. Wala kang ipapasa na oath form sa mass oathtaking ceremony sa SMX. At tama, mag te-take ka muna ng oath, tapos ipapasa mo ang oath form sa Ocotber 20 sa PRC.
3. Korek. Di mo kailangan magasikaso ng kung anu-ano. Iintayin mo lang ang araw na kung kailan ka dapat mag-register sa PRC. Para lang mas malinaw: Ang oathtaking sa SMX at ang initial registration sa PRC ay dalawang magkaibang bagay.
4. Wala kang isasubmit bago ka pumunta sa SMX. At tama ka ulit, malamang yun na ang validation mo na umattend ka ng oathtaking kung bumili ka ng ticket.

Does the consulate in a foreign country talk with the PRC? Rona, let’s say I got my registration number over at the consulate. But I have to let PRC sign some forms in order for me to facilitate documents required for nursing abroad. The form includes date of registration, its expiry, and of course the number. So my question is, will PRC be able to know that sort of info since I registered abroad? I hope you understood my question.
By the way, what do we write in the oath form? Nursing? Nurse? Nars? it’s all in Tagalog so I was thinking whether I should also fill it out in Tagalog.

Ask ko lang po kung ano yung isnesnail mail ng prc sa amin? Andito po ko USA for vacation at magooath sa consul, at may nirereuire na certificate from prc na nakapasa ako. Pwede na po ba yung imemail ng prc ang maging proof ko po?

@Ervin – all your documents would be coming from the PRC. The consulate’s purpose is to represent the PRC for you when you take your oath abroad. In the oath form, you can put either “Nurse” or “Nars”. No issue if it’s in Tagalog or English.
@Maine – I am not 100% sure if what would be snail-mailed to you would suffice as a certification for the consul that you passed the board. Is there any way that you can ask them about this? I will try my best to look into this though. Medyo madami din kasi kayo na nasa abroad na.
@Rob – ang “Roster of Registered Professionals” ay ang listahan ng PRC ng mga pumasa sa board exam na binigay nila. Oathtaking ay ay ang pagsumpa sa Board of Nursing. Initial Registration ay yung pag rehistro mo sa sarili mo sa PRC bilang isang propesyonal.
