PRC Initial Registration Instructions for July 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2010 – 2013.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)

Can we do this even before the oathtaking?
Hi Ervin. What usually happens is that the PRC will post the initial registration date before oathtaking.

Yes, magkaiba sila:
1. PRC (Professional Regulation Commission) license card (450).
2. PNA (Philippine Nurses Association) membership card (400).

Schedule of initial registration in PRC Manila has yet to be released. Expect it to be announced before the oathtaking.

@arxc – Not necessarily. You can still get your PRC license even if you won’t be able to attend the oathtaking ceremony on Sept. 20 or 21.

@ren – Yah pwede na bumili ng tickets para sa oathtaking kahit hindi pa registered sa PRC. PNA has a list of the successful examinees of the July 2010 nursing board exam. They will refer to that list during ticket selling.

Is it possible not to attend the scheduled date of the oathtaking on the 20th and 21st. I heard it is possible to do it elsewhere and on a different time. What information do you have about this? I am trying to get personal documents prepared as soon as possible.
And I am sure PRC accepts documents required in international processing. I have to let them sign something in order to complete the application package I have in another country. Where should I go?

@Ervin – yes you may not attend the oathtaking on the 20th or the 21st of Sept. You can do it at the PRC.
Walang ticket na babayaran for PNA. Bibili ka lang ng ticket PARA SA OATHTAKING sa PNA office.
Ang babayaran sa PNA ay ang membership mo sa kanila. Iyun ay kung gusto mo maging miyembro ng PNA.
@cheska – Ticket price for oathtaking is 350 for inductee and 200 per guest (only 2 guests are allowed). Kung gusto mo maging member ng PNA, bayad ka din ng 400 membership fee nila. Kung hindi, pwede din na hindi ka magpamiyembro.

@khaye – kapag hindi nakapagregister, wait ka na lang hanggang matapos ang registration schedule na binigay ng PRC. Actually, pwede ka naman magregister kahit kailan mo gusto. Nagbibigay lang ng schedule of registration nag PRC para hindi sabay-sabay pumunta ang mga bagong nurses. Dami kasi natin eh. Hehe. Take note that the PRC doesn’t allow you to register if it is not your day to register yet, though.

@cheska – based on past registration schedule, sa mid-October pa ang letter A. I should know kasi letter A din ang first letter ng surname ko, hehe.
Matatapos ang registration mid-November. Halos 1-month.

@Marian – yes sa PRC na mismo. Doon naman dapat talaga kasi sa PRC ka naman manunumpa. Kung sa PRC, di na kailangang na naka-gala or cap. Any formal attire will do.

Yes pwede ka mamili ng schedule mo.Mas maganda kung makabili ka ng maaga para makapili ka. “First-come, first-served” basis kasi ang bentahan ng ticket sa oathtaking.Hindi ka pala pwede mamili ng schedule sa oathtaking. Your oathtaking schedule will depend from what school in the NCR you are from.

Update: 1 Sept. 2010: Kung sa Baguio ka nag-take ng board exam, walang kaso yun kung gusto mo mag attend ng mass oaththaking ceremonsy sa SMX. Pwede din naman na hindi ka umattend.
If first come first serve yung sa oathtaking, would it be possible na maubusan kami tickets?? So this means I should buy earlier?? May deadline po ba yung buying of tickets? Are they selling it now sa PNA? Thanks.
@Andie – yes it is possible that tickets may run-out. I recommend that you buy them as early as you can.

Ticket Date and Time: September 20, PM schedule – will be sold until September 20, 12 NOON
Ticket Date and Time: September 21, AM schedule – will be sold until September 20, 5:00 P.M.
Ticket Date and Time: September 21, PM schedule – will be sold until September 21, 12 NOON
This ticket-selling schedule applies until all tickets are sold out.

@Jerick – not sure if you can ask the PRC for the GWA (General Weighted Average) this week. When asking for it, you do not need to bring anything. You will just have to fill-out this 1/4 piece of paper from the PRC with your name. I-pa-pass mo lang tapos susulatan na siya ng taga PRC ng mga grades mo.

@Jerick – Usually makukuha ang PRC license ID card and PRC certificate 2 months after the initial registration date.

Hi, RN here but working currently as a call center agent here in Makati.
Graduated 2009, took the board and passed the June 2009 exam.
Before PRC released the list of passers, I’m already an agent. Due to my shifting schedule, I wasn’t able to attend the Oath Taking at SMX. I knew it already but I am too busy and had no chance to register. I went to PRC and asked for my ID and they gave it to me, after 2 weeks.
Here are my inquiries:
Is it okay not to attend the oath taking? If not, can I join the oath taking of the new passers for formality sake?
Since I have my PRC already, do I need to accomplish as well the PNA? PRC ID has a validity date, how about the PNA?
Regarding trainings and seminars, on which website do I need to look into so that while I’m working, I can attend trainings and seminars as well.

1. Yes it is okay not to attend the oathtaking ceremony. But if you want to join the oathtaking ceremony on Sept 20 or 21, you may do so.
2. You do not need to be a member of the PNA if you don’t want or need to. PNA membership can be for a year or “lifetime”.

I passed the nle last Nov 2009, i was able to attend the oath taking, but hndi pa po ako nkakapagparegister til now, when ok lang po ba kung magparegster na ko? Or kelngan ko pong sumbay sa sked ng mga passers po ng july 2010? Thanks a lot…

Regarding the trainings, some of them are asking for pre-requisites. Like for Life Support etc.
And they are costly..
@Roland – You’re welcome. I agree, some training are costly. You will see that there are differences in the amount even if the training are the same. This is one of the reasons why I am compiling these seminars and training — to give fellow RN’s choices.
@sai – Pwede ka na mag-register kahit kailan kasi long overdue ka na, hehe. Most preferably before the start of initial registration schedule of the July 2010 nursing board exam passers para hindi hassle. Di mo kailangan sumabay sa schedule ng initial registration nila.

Does PRC give out official envelopes just in case you need the applicant needs it for other matters?
