PRC Initial Registration Instructions for July 2010 Nursing Board Exam Passers
Professional Regulation Commission (PRC) instructions for new professionals who have just passed the nursing board exam (Nurse Licensure Examination):
Initial registrants should come personally to the PRC office to affix his/her signature in the registry book.
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or “Panunumpa ng Propesyonal” form. (This Oath Form is also available at the PRC.)
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with your NAME and PROFESSION (Nurse) written on it.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Triennial Registration Fee of PhP 450 (four hundred fifty pesos) for 2010 – 2013.
INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal” form.
Step 2. Pay for two sets of metered documentary stamps (one stamped on the Oath Form and one for the Registration Certificate) at the Customer Service Center.
Step 3. Present duly-accomplished Oath Form (where your profession belongs, for verification of records and assessment at Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.)
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled (usually after 2 months upon registration). Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the application has been filed. If there is an unavoidable need for a representative, the representative must present his/her identification paper with picture and a Special Power of Attorney.
IN CASE THE REGISTRANT IS LOCATED ABROAD:
1. The following are the requirements:
- Take the professional’s oath before the Philippine Consul.
- Special Power of Attorney notarized by the Consul.
- Copy of passport page showing personal data.
- Copy of passport page showing date of entry abroad.
2. Refer to PRC Resolution No. 2006-313 Series of 2006 (Resolution amending item No. 1, Paragraph 3 of Commission Resolution No. 2004-235 Series of 2004 regarding the personal filling up of entries of data in the registry of professionals.)

san na po ung certificate of registration dun?
just wanna ask lang po.last year kase ako nagfile nang registration sa PRC. and still hanggang ngayon hindi ko parin nakukuha ung PRC ID ko. nasakin pa nman ung claim slip. makukuha ko pa kaya ung Id ko or do i have to pay para magpagawa nang another one?ty po.

@jhiezelle1 – the latest schedule is what you would follow. no changes to your schedule if you are not affected by the latest update (V-Tech registrants).

@mariel – as far as I know, PRC doesn’t allow a representative to register for a registrant during initial registration. Unless you are in another country. You may call PRC to verify this: http://www.philippinenursingdirectory.com/professional-regulation-commission-prc/
juz wanna know lang kung anu2x ang mga requirements pag nagapply ng NLE sa prc baguio wherein, ung previous application ko is in PRC manila..for 2nd takers po..wat to bring in PRC baguio if ever..tnx
@mel – requirements are just the same like the first-time you applied. Except you won’t be submitting anymore your RLE and OR and DR cases: Nurse Licensure Examination (NLE) Requirements (First-Timers and Re-takers)
ask ko lang po. Kase kakaregister ko lang kanina then nagkaroon p po ng problem dun s 1st name ko. Ang full 1st name ko po kase ay jay-ar with hypen. Tpz ang nlagay ko dw dun sa PERRC ata yon ay without hypen tapos dun naman dw sa DATABASE with hypen naman. Anu po dapat kong gawin. Salamat po.

@Mj – if you request your certificate through PRC’s express service, you may get it sooner than their regular PhP 75 request of certificate transaction. I just am not sure how long it would take. hi ms rona. regarding sa express service ng certificate of passing request na kailangan ni MJ, dapat andun sya ng maaga. ung tipong before 8am nakapila ka na. kasi 200 requests lang for the express service ang tinatanggap ng PRC nung last time na andun ako. pag nagexceed na sila sa limit, dun ka na sa regular processing and that will take 2wks (in my case) before release.

hi ma’am rona! im done with the initial registration yesterday in vtech. share ko lang, got there 10ish, lined up then inabutan kami ng lunch time 12-1 then resume with the same line and got done before 2…will get mine on the 21st.

ma’am rona, another simple question from a simple boy like me, me and my girlfriend is applying for NCLEX-RN…is it okay if we send 2 separate envelopes (sealed with our TOR) but only 1 package since there’s only 1 destination…so we could split the bill…or we really have to mail it separately? because the thing is, schools give our TOR back to us but we have to make it appear that the school sent it to the state… and how much does it cost for the mailing of a particular page which the state requires to be verified and sealed by PRC..thanks ma’am rona!

That’s how it usually is done. Our schools won’t pay for the TOR to be couriered (cheapskates they are). They’ll just seal the envelope with the TOR inside and you will have to send it through your preferred courier service. It doesn’t matter if there’s a bunch of separately-enveloped TOR’s in one package. The important thing is that the envelopes are intact (school seal is not broken).

question ma’am…. i already sent the application form to ARKANSAS, then PRC paper and the TOR will be mailed maybe first week of June. is that okay? i mean the application form went ahead…and is it okay if i take the exams in MARYLAND even if i passed the requirments from here… because i will be going there early next year so i plan to take it there narin..will there be extra payment..thanks!!!!

ma’am rona i’m sad… sabe daw arkansas board of nursing already requires SSN to get the license. im not sure if it also means i cannot take the exams anymore without having SSN. the problem is, i already sent the payment with the application form plus the NBI fingerprint card plus payment..i believe its non refundable..i dont know what to do then..haaaaaaaaay
@2by2boy – yes unfortunately, an SSN is a requirement now before you can take NCLEX. Wala na sinabi sa’yo sa board of nursing website that it is now a requirement?
hi ms. rona, may certficate ba na pedeng irequest sa prc stating my rank/place during the board exam? and can i request one even though di naman ako kasali sa national top 10 but instead sa regional top 10 lang.

@bin – I am not sure I have heard of replacement request of a nurse certificate of registration. by replacement request, did you mean just a request of a certificate of registration?
hi. tanong lang po. di po ako nakaattend ng oath taking and di pa nakapag file ng kahit ano dahil nasa U.S ako. nabasa ko po yung dapat gawin incase nasa abroad, yun ang dapat sundin ko, but there’s been a change of plans, babalik po ako dyan sa philippines, by any chance, pwede pa po kaya ako makapag oath taking and makuha license ko? ano po kaya mga gawin ko? thank you!
@helphelp – you will just have to go to PRC for your initial registration just like how you were supposed to on the date that was assigned for you. The PRC will also take care of your oath-taking.
hi po Ms. Rona, ask ko lng po kasi di ba may nurse registration sa OCHS this week? iba pa po ba yun sa initial registration na sa PRC iaacomplish as posted above? or same lang? pag nagawa ko na ba ung sa PRC do I still need to come at OCHS? and kung hindi aattend ng oath taking, kailangan ba nakagala uniform p dn kht s PRC lng? thanks.

@tonet – you will have to claim your Certificate of Registration in the PRC office where you initially registered.

@Brux – depends with PRC’s processing. Sometimes less than a week, sometimes more than a week or month.
mam, i left philippines after taking the july 2010 nle board exam. i passed the said exam but i couldn’t register personally at the prc as i’m tied up here in abu dhabi, uae. i have asked my sister as well as my friends to inquire from prc if it’s possible for me to appoint a representative to initially register on my behalf (after following the said requirements and conditions such as going to phil.embassy, power of attorney, etc). however, for 4 consecutive times and on different occasions, they were told that it wasn’t possible and prc personnel (in zamboanga where i took the exam) insisted that i should register personally. ilang beses na po ngtry yung sister ko at kahit yung mga friends ko na pinakiusapan ko eh ganun din ang sinagot sa kanila. is there any way to solve this? thanks in advance.
@ben_uae – Unfortunately, there’s nothing to solve. I haven’t read anyone saying that they were able to register at the PRC through a representative. Sa renewal ng lisensya pwede pa pero ang pagrehistro for the first time, wala pa. You will really have to register personally at the PRC since you have to physically sign the book of roster of professionals. There’s no other way to do that but to come over to the PRC yourself.
Thanks for the reply mam. I have read Resolution No. 2006-313 Series of 2006 several times which should particularly refer to my case. I just really wonder why it can’t be applied to me. It’s not so easy to travel back home to Philippines with an overseas job contract that needs to be completed first. Tapos kailangan ko pa rin po gumastos ng malaking halaga for my fare back home para lang makapagregister at makuha ang license. It sounds very impractical. Ok lang sana po kung vacation ko na kaso next year pa ako pwede umuwi at makapagbakasyon. Gusto ko sana makapagregister na at makuha na tuloy ang license ko ng maaga to seek for a greener pasture here. Kaso, learning from your reply it seems next year pa ako makakapagregister nito in time for my vacation leave. Pero di po ba maliwanag naman ang nakasaad sa PRC resolution, to quote:
“This PRC resolution is for new Filipino professionals (those who have passed or recently passed board examinations) who left for abroad immediately after taking their board exams in the Philippines or before the exam results were released and wants to register with the PRC.
Briefly, PRC Resolution No. 2006-313 Series of 2006 is to allow Filipino professionals based outside the Philippines to apply for initial registration with the PRC.
Prior to this resolution, all professionals had to personally appear at the PRC to fill-out all entries and affix their signature in the registry book during initial registration.
Filipinos who have passed or recently passed board examinations in the Philippines and have left for abroad without registering their names at the PRC Registry of Professionals can file for initial registration through their duly authorized representatives subject to the following requirements and conditions…”
Pwede ko naman po sundin ang mga requirements and conditions if only I’m given the chance. I really hope someone has done this before successfully and be able to relate his/her experience to us.

Gaano ba katagal ung verification of registration , nag file ako last aug.8. my problem is di nagrrerply ng PRC CDO sa quiry ko,sabi kasi ff.up after 3-4 weeks.eh dipa sa ngrereply.at PRC na kasi mghuhulog directly sa Malaysia Board Of Nursing.
