DOCUMENTS NEEDED:
1. Accomplished registration form. See below.
2. 2 x 2 ID picture with white background
3. Updated Curriculum Vitae
4. Photocopy of your PRC ID (bring original for authentication)
5. For nurses, a photocopy of your PNA ID
6. A P1,000.00 deposit to any BDO branch

If the payment was made through BDO, please inform ADNEP via SMS or fax the deposit slip with your name to (02) 942-0313. The PhP 1,000.00 deposit will be deducted from the total PhP 9,000 training fee upon full payment. If you decide not to pursue the training, only 500.00 will be refunded to you.
