@milded – no the oath form is not a requirement in the mass oath-taking ceremony. You are going to submit that at the PRC only.
kase last feb. 21 ngpunta kme prc, kumuha oath form, this monday kmi babalik march 7, tatanung ko lng hindi na ba sa prc manila i-aacomplish ung form , un nb ung sa Quezon City??

Hi mam, ask ko lang po kasi my schedule for registration in cebu is earlier than my date for oath taking (Oct 1, Bacolod kasi ako). Di ba naka pag pass na ako ng oath form upon registering, do i need to do an oath taking pa po on our scheduled date na October 1 together with all the bacolod passers? thanks po!
@johanna – it’s your choice if you still want to attend a ceremony. You have already submitted a signed oath form to the PRC, that itself is taking an oath already. What else do they want from you, recite it?
Eh pumirma ka na nga sa oath form. Oath taken. Yun na yun. 
mam Rona, ask ko lng po kung may required attire pa po ba pag halimbawa sa PRC na po aq directly manumpa? and pde po ba sa mismong date na lang ng pgpaparegster manumpa? sa QC extension po kc aq..
@mary – somebody commented here that the dress code during the PRC’s oath-taking ceremony is still the same as the one in SMX — complete gala attire. Read my reply (above this comment) to johanna regarding your second question.

@mike – there are forms that you have to fill-out and affix your signature with when you apply or re-apply to take the NLE. Verify with the PRC if a representative can do this for you. Their numbers are on the right side of this page. For repeaters, the steps on how to apply to take the NLE is just the same when they applied for the first-time and the fees are still the same except the RLE and O.R./D.R. cases won’t be submitted anymore. Having said that, I assume you know now what to do since you already have applied to take the exam before.

mam rOna isa pang questiOn,.pacencia napO kau,..mam rOna ung oath fOrm pwede ku pu ba cyang panOtarized na sa labas,..para pu mas mabilis,..kUng dun pu akO sa vtech magpapanotarized,..ibang bayad pa pu ba un or kasama napu cya sa 1k plus na magaagstOs ku don,..ang metered stamps and brown envelope pu ba available na pu doon,…dko pu kc alam kung may contact number ang vtech e,..salamat po sa pagreply,.. =)
@october242007 – yes pwede pero kung sa V-Tech ka magre-register hindi mo na kailangan ipanotarize according to those who commented here na nakapagregister na doon. Sila na ang bahala mag-notarize ng oath form mo.
Hi po ma’am Rona RN pwede po ba magtanong?Uhm pwede ko po ba mahabol yung PERCC no.,at signatures ng records officer sa claim stub(for the license card) kasi ngaun ko lang napansin po na blanko pa pala ang mga yun.Pero nakapagregister na po ako last sept.6 2010.Pwede ko po ba makuha yung license card kahit wala po ang mga yun?Tnx in advanced po

@Yes23 – I think hindi lang nila nilagyan yung mga linya na yun. You can always verify with PRC if your records are appropriately filled-out.

SPA’s are only needed for registrants who already are abroad. You can verify with PRC the rest of your inquiries.

hi maam rona. quick question lang po. i’ll be taking the board exam this dec 2010 here in davao. After the exam, lipat na po ako sa Manila. Pag pumasa po ako, pwede sa Manila na ako mag register, apply and mag oath taking? kahit taga Davao po ako? Thanks!
@pulpy – as of this writing, PRC has no official announcement that a test-taker’s file can be transferred from one PRC office to another. Their “file where you applied” policy still applies. As for the oath taking, you can take it anywhere you want to.

Mam, I was able to read the site you gave me. But my classmate is going to PRC tomorrow for her Registration. Is it possible to jump into Registration already even if she didn’t take the oath taking first. kasi po the one i’ve read imply that oath taking should be accomplished first before I can go for Registration. I’m a little bit confused po.
@MiraFlor – Really think about what oath-taking and registration is. During oath-taking you are promising that you will do due diligence as a registered nurse. You sign that oath form and naturally, the next step is to process your application for a license. it doesn’t make sense if it will be done the other way around. BUT, LAST YEAR (July 2010) it did happen that some nurses were scheduled to register at the PRC first before the oath-taking ceremony at the SMX. That doesn’t matter though because the “REAL” oath-taking happens when you SIGN that oath form and submit it to PRC during your initial registration day.

@nico – Wala eh. Pero promise, hindi ka magkakamali niyan. BASIC personal information lang naman ang isusulat mo sa oath form. Check mo sa taas kung ano ang mga iyon. Mas madali nga siya sagutan kaysa isang raffle coupon. Kaya mo ‘yan.
