@cyrille – Yes that’s correct. Fill-out mo na siya bago ka pa pumunta sa PRC para di ka nahirapan magsulat-sulat doon. Meron din silang ready-made na form doon though.

Sec. 16. Oath. — All successful candidates in the examination shall be required to take an oath of profession before the Board or any government official authorized to administer oaths prior to entering upon the nursing practice.
It means that new R.N.’s are mandated by the law to take an oath. However, that doesn’t mean that the mass oathtaking ceremony on Sept. 20 and 21 at the SMX is the only way that you can take your oath. You do not need to pay 350 pesos just to take an oath. You can do your oathtaking at the PRC and you do not have to pay for it. The 600 pesos Initial Registration Fee that you will pay at the PRC will cover for that.
THAT OATH FORM or “Panunumpa ng Propesyonal” Form — when YOU HAVE SIGNED IT, and the “PINUNONG TAGAPANUMPA” and THE “KATUNGKULANG OPISYAL” HAVE SIGNED IT, YOU’VE HAVE ALREADY TAKEN YOUR OATH AS FAR AS PRC’s STANDARDS ARE CONCERNED. It’s really all up to them if they would bother giving that mini-oathtaking ceremony in one of their small rooms.
Hindi ka magpapamember sa PRC, magreregister ka lang bilang isang propesyonal. Sa PNA ka magpapamember, at yun ay kung gusto o kailangan mo lang.

@chi – attend the oath-taking ceremony and go to the PRC on your scheduled date to register. If your day to register comes first before you can take your oath, it doesn’t matter. the oath form is available at the PRC all the time.

@Bankai – I can not give an assurance that they will allow you to register AHEAD of the day that you are supposed to. Your papers will just be in the PRC. It would be okay to register on a much later date.

@Lainey – saan ka a-attend ng oath-taking ceremony? Kasama na sa proseso ng pag-rehistro mo sa PRC ang pag notaryo ng oath form. The list of requirements during initial registration doesn’t say that the oath form should be notarized before you get there.
@cristina – wala pang schedule ng initial registration para sa Davao. May ibang seminars and training na di kailangan ng PRC license. Like BLS.

@Mary – what stub are you referring to? What the PNA need is your SEQUENCE number in the official nursing board exam result list. It would make it easier for them to look your name up.

@Nikko12 – You can ask somebody to get it for you. You might want to read this resolution of the PRC: PRC Resolution No. 2006-313 Series of 2006. It has something to do with the personal filling up of entries of data in the registry of professionals BUT it may shed some light for you if you are just aiming to get your “certificate of passing”.

@jelly_boy – you do not need to get your exam grades to register. The PRC KNOWS your grades. They do not need it from you. In fact, YOU need it from them. Documentary stamps are available at the PRC.
@bab – you can take your oath anywhere you want to. I have no information yet regarding the schedule of registration in Iloilo. I will post it here when it has been released by the PRC.

@Wasted – you are not going to process your oath form. you will just fill it out, sign it and submit it when you are registering to the PRC.

@Jay – Yeah ididikit siya sa oath form. I’m sure one of it will be used for your PRC ID. The rest of the pictures are for PRC’s files.

@toni – yes to your question. no need to present anything at the PNA to buy tickets for the oath-taking ceremony.

@toni – unfortunately, the PNA is the only place you can buy the tickets from. Unless your school has bought the tickets for their former students.
