do these forms do not warrant signatures of the chief nurse of the hospital where they got the said cases?
hi,
Just wanna ask if the above forms are the ones that should be used for the OR/DR cases? just graduated last march (class 2011) and im just a bit confused about what form should i use. even my school could not give me concrete instructions about the said forms. i tried to call the prc hotline numbers listed but sad to say i could not get any one to attend my concerns. hope you could help me. i graduated under the curriculum mandated by CMO 14. thanks
@neil – it seems like these forms differ per school. This can be your guideline but I can’t give a 100% assurance that this is the format that is currently being used.
@neil.. it is really confusing on what form to be used.. but last year they’d used the form as shown above and at the beginning, we were instructed to use this but there is a rumor that PRC had issued another new form for cases. i do not know how true this is.. as Rona RN says,, there is no guarantee that this can be the form we will use. As i search the web, many forms are being used by different schools.

Hello. Regarding the OR cases, are “excisions” still allowed or are they considered diagnostic procedures? The exact diagnosis is “Excision of Ganglion Cyst, right hand.”
yes,considered minor surgery pdin po ang excision..
i applied to ca board of nursing i have submitted all complete documents. But i recieved a letter needing copies of minor major cord delivery cases. does prc have a copy?
can I ask a question is a laparoscopic cholecystectomy is a major or minor cases and do we need to submit minor cases kasi po the school said that the student under the new curriculum ay major ,actual delivery ,circulating and new born care lang po ang isusubmit sa prc?

what if 5 years na pong graduated but until now d pa nkakapagtake ng board..ilang years po ba pwedeng iconsider ung cases? or kailangan back to zero..hope for your response
